What’s medicine and healthcare like in your world?
I actually reeeealy tried to dodge this one. But then one of my o.c.s went into nursing and its like... welp... lets make a system.
While it varies from region to region Kanto and Ithica are where most of the action hits so heres the breakdown.
Kanto.. is a mix match of privitized and public. But the government plays favorites.
Who gets what
Kids, parents of minors, trainers, and public authorities get free healthcare. Where they get access to varies by sociatary value points.
There's a sliding invisible scale here, an actual point system thats a mix match of income, status, activites, ect. Its like a morality scale.. except Kantos ' gov set the slider and thier laws/norms set the pips.
But in order to be on the scale you have to have a certain amount of Kanto ancestry.
As a base rule, if you arent of kanto descent and get hurt/sick you will be actively.denied all services... unless you pay. Being a minor wont save you, actually, you could get shipped back to "where you came frpm" if its sever enough... and you won't die in transit. But as a rule of thumb, foreigners have to buy insurance coverage, or they're dead out of luck.
All italian/Ithica expats, from the last Great Purge, who persisted and whose ancestors squared things up legally w Kantos gov(aka they surendered and payed fees/tithes to stay on Kanto soil), are given kanto civ status but with a hit on thier base score.* War Vetrens of Italian/Ithica decent who served/survived in active combat can pick either nutrality for thier own status and that of thier spouse parents (starting them at 0), or have it applied to thier children. One or the other. Not both.
People start as high priority as a minor. This ticks down as you age out into adulthood. By 20-25, you're likely coasting at a neutral 0, your actions/jobs/contributions can scale you up to a higher value. Being a high-profile anything with a good reputation or international appeal can keep you in the high status so long as you stay relevant.
why score matters
anyone with less than a 0 score is not getting services (low scores also have other consequences, job prospects, transport options, trainer status, ect). Theyll be kicked out of hospitals, denied transport, specialty services, anyone who treats them is risking fines at best and thier license at worst.
The higher the score the better services you can get. Someone with a 0-5 for example can walk to a gen practitioner office, get thier fku shots, get basic meds for colds flus, ecf.
But they aren't going to have access to say, newer treatments, mental health services, or say cancer treatment.
(Theres some workaround. Basically, "throw money around" can open some doors, but not all.)
Scores, access personages and perks.

Base healthy citizen, job holding, confirmed kanto citizen of 3/4s kanto heritage, or military service bought.
Most services are free. Mostly, meds are the exception
1-5. Base healthy young adult, job-holding or low-tier trainer,

Fees start appearing as free services are cut. Bills can range from low to high depending on the services needed.
6-10
(Afluent, optional) , healthy, job-holding, some service rendered either abroad or locally that has impact, low to mid tier trainer.
Basic medical treatment, multi check up, dental simple to moderate service. Lifetime one use of complex (ex: denture), minor sergical overlap multi times a year (dental surgery, setting broken bones, appendex)... main goal is to get them back up and at 'em asap.
Fees range from low to high depending on services.
11-15
(Alfluent, optional) health, job holding, regular services rendered either abroad or locally, low-tier gov worker, mid-tier trainer
Same as before except specialist help is available (ex. mental health,). And if you miss access to a service per it say being classified as a 14 and you are at an 11) it's possible to buy your way up in this bracket, but not out of it.).
Fees can be high.
16-18
(Possible to pay in with score of 11+)
Afluent, serious clout either abroad or local. Keystone service/duties implied (e.g., firefighter, police, doc, businessman)
Almost all services are open; some exceptions are experimental and exotic/holistic. Fees are outrageous.

Almost 100% access to anything available. Some exceptions are cutting-edge gene editing therepy (more to Kanto not having resources to offer the services than any other reason) and the more exotic services... like the healing pools in mountmoon, certian paths in Viridian forest meant to heal, ect.
Fees are astronomical.

President of kanto, head of mon research, head of interpool/police/army generals /commerence commitee upper tiers.
Access to anything and everything, as well as INTERNATIONAL services. free of charge.

(say pickpocketing occures and you pay the fee off, this will still leave a permanent knock on your totals)

research or counterband on taboo topics (this can range from talking about/reading up on scandles on people in the up and up, or simply atrocities that Kanto wants to keep quiet, to owning books Kanto doesn't want you to own)

Special notes: as a ward of Giovanni Sakaki it is noted that she can be offered higher-tier services beyond her score, but all bills/incurred finances are to be charged at 200% and must be funneled through the legal avenues of ViridianCo' before crossing her desk.
Don't want to play a number game with repute, here's the alternatives
Got a family member in the slammer, probable Rocket supporter/member? Well, it's not instant death to get sick... but it's not easy to get better.
For any at a score of 0 or lower, all official government-sponsored facilities, including pokemon centers, will be unavailable. However, there are a slew of privatized clinics seeded through Kanto, ranging from small-scale hospitals to mom and pop minute clinics for human and mon. But getting treatment is a gamble.
Because they are not government-supported, they do not need to uphold to governemnt clenliness standards, as well as quality levels for provided meds ect.
They also must charge ALL patients fees for services (otherwise it's a "charity" which is a government-controlled asset at that point).
Clinics usually bill the poorest a penny, then pull a penny out of the tip jar to cover it. This is why all doctors' offices and even surgical/ emergency areas have a tip jar.
It's not unheard of for someone to be hauled in catatonic, fingerprinted in steed of signed in, and the facility takes that as a signature, punch the penny code in, then starts treatment/ in a few cases it's been done while the patient is being cut into/knocked out, for surgery, gettign treated for a gunshot wound, ect.
Suffice to say, hospitals and centers (government-sanctioned medical treatment places) do not get a large influx of poor, rocket, or international visitors.
Clincs on the other hand, are usually populated by people in the Game, with criminal histories, homeless, or in low-income jobs.
hospitals/mon centers and clinics are not obligated to share information with each other (which ended very badly during thier last pandemic).
The patient, if they luck into a higher tier of service, would have to request a transference of data if going up. Those downscaling usualy wnat to get thier paperwork in order before scaling down, because once the lower scale kicks in, they can be blocked from all services, even records (in case of say a family member getting jailed).
Because clinics are not gov sponsered its the discretion of the medical team there if they will pass up paperwork about a client on government/police demand. Most clinics, out of sheer spite, will destroy a client's paperwork who is being investigated for being associated with a rocket before passing it up.
Staffing between facilities:
Gov hospitals/mon centers have thier staff regularly tested psychologically, physically, and certifications must be re-earned every 5 years. If you fail on one of those three levels, you are stripped of the ability to serve until evidence is provided that you're capable of coming back to work with no risk. Skill and professionalism are paramount, and any lapse on either front will lead to demotion/phasing out.
Clincs will vary thier criteria on a facility-to-facility basis, they will take in those with lapsed licenses, shoddy history, or those who may have been failed for say, hitting too high an anxiety scale. There's a sliding scale of skill based off of geological location and people willing to tolerate the conditions.
Those working in either or need to pass personalized tests of the established depo heads (and gov tests as well for hospitals), once they do they're worked into scheduling needs and whatnot. Someone double-dipping (working both Clinics and Hospitals) will definitely be considered for positional re-review and slated for more blood tests for "everyone's safety" in the government facilities.
Tells:
Clinics are named after catholic saints, there's usually some backhanded humor in the name (Cinibars, for ex: is named "Saint Lawrence".). The staff usually are bilingual, uniforms are bare basics, and you'll see some catholic iconography and even the occasional chapel in the larger more well-off facility. Supplies for staff are minimal (and usually supplemented by either a local capos donations, or local businesses, depending on the criminality of the area). People can be seen visiting relations in full Rocket regula, and there's very rarely police or government presence tolerated. it is considered taboo to perform any type of sting operation, and info gathering. Clinics are neutral territory, and there's a common story about how a mobster with a knife wound nd his mark (missing said knife) will troop in together, supporting each other, because alone they couldn't walk and they really need treatment. Both are perfectly civil the second they step into the door, until the robbery victim demands his knife back and the Rocket tries to get the medics to stab the man with the bill of it's retrieval per contamination hazard. Stating "Karam's a bitch you stabbed me, I'm stabbing you back".
Hospitals: are ususaly named after colors and numerized. There's usually some arciean worship symbology in the architecture/logs, and the uniforms and supplies are top-notch. Theres very much a lot of pressure for everyone to blend in/look the same. Outside resources are shipped in regularly (rotating translators, comforters, and actual medical paraphernalia). Police and government officials are regular faces. All children are defaulted to hospital care.
*Ithica, out of sheer spite, has universal care for all including visitors and found refugees/lsot trainers. Thier services are dodgy per poor resources, and lean more on developmental/mental health services than kanto (which ingogres therse things) a more "preventative care emphasis" than a "fix it after its broke idology.
